Pathways to Housing PA serves people with complex physical and behavioral health needs. Many of the people we work with have serious, chronic, and untreated medical issues that have often been neglected for years and require significant healthcare coordination.

To address the level of medical need, Pathways formed our Integrated Care Clinic. Our goal is to ensure a person-centered approach to care that emphasizes a welcoming environment, recovery, wellness, trauma-informed care, and the integration of physical and behavioral health care.

Pathways, in partnership with the Thomas Jefferson University Department of Family and Community Medicine and Project HOME’s Stephen Klein Wellness Center, provides a comprehensive array of services to improve access to physical and behavioral health services.

We offer primary medical care, psychiatric services, behavioral health services, nurse care management, peer-led outreach, care coordination, and assistance with applying for health insurance benefits. The coordination of medical and behavioral healthcare with on-site staff psychiatrists and the development of a system of population-based health and wellness management is also part of the Integrated Care Clinic team’s responsibilities.

The team provides support of individual and group health and wellness programs in coordination with clients and staff. Their services include health education, health screenings, chronic disease self-management support, and illness-prevention immunization programs.

We believe that housing is a medical intervention, and we are making a huge impact on quality, access, and cost in the health care arena.
