A Look Inside Our Integrated Care Clinic

At Pathways to Housing PA, we work with participants who face some of the toughest barriers to health and stability, like untreated medical conditions, mental illness, addiction, and years of housing insecurity. For many, navigating the healthcare system feels impossible. Long waits, confusing paperwork, stigma, and lack of trust often keep people from receiving the care they need.
That’s why we created our Integrated Care Clinic.
The clinic meets people where they are, literally and figuratively. It’s a place where participants can access medical and behavioral health services in a space that feels welcoming, safe, and familiar. Our approach is person-centered, trauma-informed, and rooted in the belief that everyone deserves compassionate, high-quality care regardless of housing status or history.
Enormous health inequities exist among people experiencing homelessness, especially those managing co-occurring conditions. Many of our participants are living with serious mental illness, substance use disorders, and/or chronic physical conditions that have gone untreated for years. Housing is just the first step in establishing stability.
The clinic is a partnership between Pathways to Housing PA, the Department of Family and Community Medicine at Thomas Jefferson University, and Project HOME’s Stephen Klein Wellness Center. Together, we’re providing coordinated, whole-person care that’s actually accessible. Participants sometimes refer to our Pathways offices as a “one-stop shop” where they can access multiple services in one simple visit.
At the Integrated Care Clinic, participants can receive:
- Primary medical care
- Psychiatric services
- Behavioral health support
- Nurse care management
- Peer-led outreach
- Help enrolling in insurance & healthcare coordination
- Health education, screenings, and chronic disease support
We don’t just focus on treating immediate symptoms, we prioritize long-term wellness. Our on-site staff work across departments to support every part of a person’s recovery, whether that’s managing chronic illness, addressing trauma, or building trust over time. A recent case study from the HealthShare Exchange found that our healthcare coordination efforts reduced emergency visits by 32% and shortened hospital stays by nearly two days.
Our on-staff psychiatrists collaborate closely with behavioral health therapists, Housing First teams, and clinic staff to provide the highest level of community-based mental health care, both in the clinic and out in the community. We aim to meet participants where they are in an effort to remove as many barriers to care as possible.
When someone has a safe place to live and a care team that listens, people can begin to heal. They make appointments, take their medication, and can think about the future. The Integrated Care Clinic is another commitment to our mission of providing homes, restoring health, and reclaiming lives.
