Position Summary:
The Community Inclusion Specialist (CIS) facilitates meaningful daytime activities for program participants with histories of homelessness, substance use, and/or mental health disAbilities as a trauma informed harm reductionist. The CIS helps cultivate stigma-free welcoming spaces for all peoples in the office and is a conduit to events in the community. Working collaboratively across clinical teams, they provide tailored supports for individuals, assist Certified Peer Specialists with group logistics, and lead outings promoting socialization between staff and participants. These outings include cultural events, museums, volunteer opportunities, and accessing other community resources. The CIS develops informative publications for participants and staff, facilitates events in the community, and builds bridges with community partners so participants can access resources like everyone else.
Essential Duties & Responsibilities
- Attend and participate in weekly department meeting
- Maintain written and computerized records, compile reports and complete other program documentation (including case notes, statistics, reports, letters, etc.)
- Compile data and reports concerning group attendance and funding
- Coordinate community inclusion internal activities calendar and city-wide free events calendar for review
- Facilitate monthly Tenant’s Meeting with a participant leader and records notes
- Develop and update community inclusion resource manual for clinical teams and community mapping for program participants
- Assist participants and staff in accessing supports in the community by utilizing the PA Navigate system
- Identify and refer participants to the Vocational Specialist as appropriate.
- Develop positive and working relationships with participants by attending Community Inclusion events
- Develop and implement community inclusion activities based on needs assessed
- Promote participant access of community resources while instilling the development of friendships and positive social connections
- Point of contact responsible for Radical Hospitality within the office
- Assisting participants in computer lab, managing lending library, providing beverage services, and striving to welcome participants into communal spaces during office hours, when available
- Manage special projects as assigned, including but not limited to, assisting with the planning and implementation of the Holiday Party and Participant Cookout
- Other duties as assigned
Essential Education, Experience, & Skills
- Bachelor’s degree and at least one year of experience serving persons with disabilities is preferred
- Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations
- Ability to work with diverse populations including clients and staff
- Strong and efficient written and verbal communication skills
- Strong organizational skills
- Strong MS Word, Excel, and Publisher skills
- Capable of setting goals and working independently
- Ability to meet with participants in their homes and in the community
- Ability to travel via public transportation when necessary
- Sensitive to cultural, economic, gender, and sexual orientation differences
- Excellent communication, organization, writing, and computer skills
- Bilingual a plus
- Valid driver's license and clean driving record

Salary: $43,000 annually
